Independent Facilitation
SELF-Start provides Independent Facilitator Services to California Center Clients interested in transitioning to the Self-Determination Program.
We support individuals with the following services:
- Creation of the Person Centered Plan (PCP)
- Assisting with the Individual Budget
- Assisting with identifying supports and services
- Assisting with creating a spending plan
- Assisting with the FMS
- Assisting with the IPP including development of SDP goals
- Ongoing services
